The Physical Therapist performs accurate and comprehensive assessments, obtains physician orders for certification and re-certification of home physical therapy services, and ensures timely communication of changes in patients’ clinical status to healthcare team members and agency staff. The role involves coordinating care planning and delivery with physicians, patients, families, and other healthcare professionals to achieve optimal patient outcomes. The Physical Therapist educates patients, families, and staff on appropriate physical therapy modalities and pain management techniques. They deliver, modify, and discontinue physical therapy modalities based on ongoing patient assessment and physician orders, and assess patients for durable medical equipment and adaptive needs. The position also includes developing home exercise programs, synthesizing data for clinical judgments, assessing and managing patient pain, and ensuring patient safety by identifying and mitigating environmental risks. Additionally, the role involves supporting cultural practices, performing transfer and discharge planning, participating in the organization’s information management program, and complying with agency policies. The use of the PowerPath EMR system, which utilizes AI for admissions, OASIS assessments, care plans, and progress notes, is integral to this role, aiming to reduce administrative workload and documentation time while ensuring regulatory compliance and real-time patient progress monitoring.
