About The Position

Kern River Transitional Care is seeking a dedicated and compassionate Full-Time Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist evaluates and treats residents, develops individualized treatment plans, communicates with families, physicians, and other healthcare team members, and maintains accurate documentation of services in the medical record. The ideal candidate is resident-centered, team-oriented, and committed to helping residents improve mobility, strength, balance, endurance, and overall functional independence.

Requirements

  • Bachelor’s degree in Physical Therapy required.
  • Proficiency with computer systems, including entering data into programs such as Net Health and PointClickCare.
  • Ability to read technical procedures.
  • Ability to read and comprehend policy and procedure manuals.
  • Ability to effectively present information and respond to questions from managers, coworkers, residents, and families.
  • Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
  • Ability to solve practical problems.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Current Physical Therapist license in the state of California required.
  • Must meet continuing education requirements in accordance with state practice standards.
  • Must maintain a license in good standing with the state board at all times.
  • CPR certification required, if required by licensure or facility policy.

Nice To Haves

  • Master’s or Doctorate degree in Physical Therapy preferred.
  • Additional certifications may be required based on facility needs.

Responsibilities

  • Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
  • Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
  • Assist the nursing department with training Restorative Aides.
  • Supervise Physical Therapy Assistants in direct resident care and resident-related activities in accordance with the state practice act.
  • Communicate regularly with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning.
  • Participate in Resident Care Conferences, Utilization Review meetings, and Rehabilitation meetings as needed.
  • Provide in-services and training programs for facility staff as needed.
  • Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
  • Maintain a safe treatment environment by following infection control, safety, and equipment maintenance protocols.
  • Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
  • Recommend and facilitate the ordering of necessary durable medical equipment to support resident independence with mobility, including splints, walkers, canes, elevated toilet seats, and grab bars.
  • Maintain confidentiality of resident information and company information.
  • Report any problems with department equipment to ensure it is maintained in good working order.
  • Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.
  • Relate positively, effectively, and appropriately with residents, families, staff, and professional colleagues.
  • Supervise Physical Therapy Assistants, aides, and students.
