Requirements
- Graduate of an accredited Physical Therapy program
- Active Indiana Physical Therapy license
Responsibilities
- Perform patient evaluations and physical therapy assessments
- Review medical history, imaging, and physician referrals
- Develop individualized treatment plans based on patient needs and goals
- Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs
- Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications
- Monitor patient progress and perform reassessments
- Maintain accurate and timely EMR documentation
- Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes
