About The Position
Concordia Visiting Nurses is seeking a passionate and empathetic Physical Therapist to join their growing team. This is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to patients. Requirements
- A graduate of an accredited Physical Therapy program with a valid state license.
- A passion for working with patients and a commitment to delivering high-quality, patient-centered care.

Responsibilities
- Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.
- Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
- Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.
- Educate patients and their families on treatment options, rehabilitation techniques, and preventative measures.
- Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
- Help create a welcoming and supportive clinic environment where patients feel comfortable and motivated.
