About The Position

Stern at Home Therapy is hiring Physical Therapists to provide one-on-one home health care to a primarily geriatric patient population in and around Newburgh, New York. The position is available as PRN, part-time, or full-time, with salary, W-2 per-visit, and 1099 contractor compensation options. Full-time 1099 clinicians may earn up to $145,000 annually based on visit volume; the full-time salary option pays $92,000–$110,000 annually. Clinicians evaluate and treat patients in their homes, develop individualized plans of care, and manage treatment with clinical autonomy. The role offers flexible scheduling and caseload management based on availability, one-on-one patient care, administrative support, and cancellation protection through the Stern Scheduling Assurance Plan. Eligible W-2 employees may receive benefits. Onboarding is described as fast, with most clinicians seeing patients within two weeks; shadowing is available for physical therapists new to home health.

Requirements

Active Physical Therapist license in New York.
Reliable transportation.
Strong clinical, communication, and time-management skills.
Ability to work independently in a home-based setting.

Home health experience is preferred but not required.

Responsibilities

Perform physical therapy evaluations and develop individualized plans of care.
Provide one-on-one treatment based on each patient's functional needs and goals.
Manage treatment plans with clinical autonomy.
Complete accurate and timely clinical documentation.
Communicate with patients, families, physicians, and members of the clinical team.
Coordinate with the administrative team regarding scheduling, patient care, and logistics.
