About The Position

We are seeking a casual physical therapist to support acute care patient needs at M Health Fairview Woodwinds Hospital in Woodbury. The physical therapist will work with a variety of patient populations, including, but not limited to, ortho, neuro, general medical, general surgical, cardiac, oncology and medically complex. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. This is a .01 FTE, casual position, requiring two weekend shifts every four weeks, with a preference for candidates willing to pick up additional weekday and weekend shifts, as available. Fairview Rehabilitation offers a broad range of services across 10 acute-care hospitals, 3 post-acute settings, and 70 outpatient adult and pediatric therapy clinics, encompassing Physical, Occupational, Speech Therapy, Audiology, and Cardiac & Pulmonary Rehab. Requirements

Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota
Basic Life Support (American Heart Assoc or Red Cross)

Responsibilities

The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation
Gathers pertinent data
Performs examination
Determines treatment diagnosis
Develops an individualized plan of care through collaboration with the patient and family
Analyzes data to identify needs and potential risks
Determines prognosis for recovery
Establishes individualized goals with patient and/or caregiver
Individualizes plan of care considering developmental level and cultural differences
Implements and modifies plan of care based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
Establishes appropriate discharge plan
Partners with patients and families to provide skilled intervention
Implements plan of care
Alters treatment/plan of care to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
