About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Physical Therapist to provide exceptional, individualized care to patients in Fort Myers, Florida, and surrounding areas. The role involves home health visits in Lee County, including Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs, and nearby communities. Scheduling is flexible, with the clinician choosing when and where to work; 5–10 or more home health visits are available per week.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support as part of a continuum of care. The position offers flexible scheduling, manageable caseloads, interdisciplinary support, and meaningful patient-centered care in patients’ homes. Certain roles could expand to full-time as the market grows.

Requirements

• Active Florida Physical Therapist license.
• At least 1 year of experience as a licensed Physical Therapist.
• Strong assessment, critical-thinking, and patient-education skills.
• Willingness to participate in weekend and holiday on-call rotations.

• Home health care experience is strongly preferred.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
