Croí Health is seeking a skilled, compassionate, and patient-centered Physical Therapist to join our growing Home Health team serving one of the following communities: Braintree/Quincy or Plymouth. In this role, you will provide individualized physical therapy services that help patients regain strength, improve mobility, maintain independence, and achieve their highest level of function in the comfort of their homes. Requirements

Current Massachusetts Physical Therapist License in good standing
Registered Physical Therapist
Minimum of two (2) years of experience as a Physical Therapist
Home health or community health experience preferred
Strong clinical assessment, treatment, and documentation skills
Excellent verbal and written communication skills
Ability to build positive relationships with patients, families, and interdisciplinary team members
Ability to work independently and manage a flexible schedule
Strong organizational and time-management skills
Adaptability and commitment to delivering exceptional patient care

Responsibilities

Provide professional physical therapy evaluations, treatments, and interventions in accordance with physician orders and the patient's plan of care.
Assess patients' functional abilities, limitations, and rehabilitation potential through comprehensive evaluations.
Develop and implement individualized treatment plans designed to restore function, maximize independence, and improve quality of life.
Monitor patient progress and adjust treatment plans as appropriate to achieve optimal outcomes.
Accurately document evaluations, treatments, patient responses, and changes in condition in accordance with agency policies and regulatory requirements.
Educate patients, caregivers, and family members on treatment plans, safety recommendations, home exercise programs, and the use of assistive devices.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
Participate in the development and ongoing revision of patient care plans.
Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs) as appropriate.
Coordinate patient equipment needs through approved medical supply vendors.
Identify patient and family needs and facilitate referrals to community resources and support services.
Participate in case conferences, community education initiatives, agency in-services, and continuing education opportunities to maintain clinical excellence.
