Requirements

NYS professional License and current registration as a Physical Therapist.
One (1) year of full time experience in direct provision of physical therapy service.
Ability to assess and respond to the needs of patients and families in the home setting.
Ability to utilize industry technology such as a tablet when in the field with patients.
Ability to drive with automobile in good working condition and available for use in the field.

Nice To Haves

Home care experience preferred.
Experience with Oasis assessments is preferred.

Responsibilities

Assess patients for functional limitations and develop a therapy plan of treatment with specific goals in collaboration with the patient, family, nurse, physician and other health team members.
Provide direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities.
Instruct and supervise the patient/family and home health aide (and others) in the home exercise program that they write and other aspects of patient care as appropriate and documents their response to instruction.
Continually re-evaluate the patient’s condition and modifies the plan of treatment as appropriate.
Identify and report significant clinical information to, physician, clinical manager, primary nurse coordinating services and other disciplines.
Document all clinical evaluations, treatments and patients response to therapy.
