Requirements

  • Bachelor's Degree in Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • Current and valid NYS Physical Therapy license and registration.
  • 1-2 years of experience in a Hospital (Inpatient), Orthopedic (Outpatient), or Pediatric setting.
  • Excellent communication (both oral and written), critical thinking, and customer service skills.
  • Basic computer skills.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • Master's Degree in Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • Post Doctoral/Doctorate in Physical Therapy (DPT) Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • 1-2 years of Home Care experience.

Responsibilities

  • Educates patient and family regarding patient's functional status and plan of care.
  • Provides a home exercise program and instructs on home safety factors.
  • Re-assesses patient progress toward short term/long term goals and modifies the plan of care accordingly.
  • Completes and maintains accurate, complete, and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention, and evaluation according to Federal, Department of Health (DOH), and The Joint Commission (TJC) requirements.
  • Assists Agency leadership with achieving corporate goals, improving quality of patient care outcomes, and patient satisfaction.
  • Participates in Agency quality assessment/performance improvement initiatives.
  • Performs an objective functional evaluation and re-assessment of patient, including the use of appropriate tests and measures, patient safety issues, and home environment factors; and establishes a plan of care with measurable and functional short term/long term goals.
  • Implements and modifies appropriate interventions/treatments as indicated based on the patient's functional status, diagnosis, age, patient response, and status in conjunction with the plan of care.
  • Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health care goals.
  • Performs other duties as assigned.
