About The Position

In this role, you will deliver high-quality therapy services to residents in assisted living environments, helping them restore mobility, function, and confidence.

Requirements

• Current, valid Ohio Physical Therapy license
• Must hold active CPR and Basic Life Support (BLS) certification.
• Reliable Transportation
• A valid driver’s license and auto insurance are required for reliable transportation.

Nice To Haves

• Previous experience in home health care is preferred.
• Geriatric experience
• Experience with care planning, discharge planning, or inpatient settings
