About The Position

FOX Rehabilitation is hiring licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in McLean, VA, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. The role offers autonomy to create and manage a treatment schedule and build lasting patient relationships. Clinicians may also provide telehealth and other virtual care when appropriate. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, and a mission to abolish ageism. The position involves working in a collaborative, interdisciplinary environment with PTs, OTs, SLPs, and Fitness Specialists. Employment pathways described include full-time, part-time, and PRN / FOX Flex; benefits depend on role eligibility. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
Ability to drive to patient homes and senior living communities.
Wear PPE when required.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
Document in FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth or virtual care when appropriate.
Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
