About The Position

Evaluates and treats all referred Physical Therapy patients according to physician orders. Home Care: Performs and supervises physical therapy programs in the homes of HFM Home Health Care clients.

Requirements

• Degree in Physical Therapy required.
• Wisconsin licensed Physical Therapist or eligible required. If eligible, must obtain Wisconsin license within 3 months of hire.
• CPR certification is required.
• Good communication skills required.

Nice To Haves

• At least one year experience in a medical setting preferred.
