They are seeking a compassionate, organized, and team-oriented individual to join their Home Health team as a Full Time or Part Time (Per Diem) Physical Therapist (PT) in Brentwood, Antioch, Oakley, Discovery Bay. This position is for an experienced, self-driven Full-Time / Part Time (Per Diem) Physical Therapist (PT) to join their dedicated Home Health team and provide exceptional care throughout the region. As a Physical Therapist (PT), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. You'll be working closely with their Clinical Director and Clinical Staff, providing support both remotely and independently in the field.

Requirements

  • Current PT License to practice in California - Required
  • Experience in Home Health or Hospice care - 1 Year Required
  • Current CA Driver's License
  • Able to travel within a 15 - 25 mile radius on a regular basis.

Responsibilities

  • Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
  • Regularly evaluates and re-evaluates patient status.
  • Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
  • Initiates, develops, implements and revises the patient's plan of care (POC).
  • Addresses all problems identified in the POC.
  • Provides educational information to the patient/family/caregiver concerning knowledge deficits or needs regarding the patient's condition.
  • Communicates with other disciplines involved in the case as warranted.
  • Maintain accurate clinical records and submit documentation according to company policy using a company-provided device.
  • Comply with all company policies and procedures to ensure high standards of care.
