About The Position
Duly Health and Care is seeking a PRN outpatient Physical Therapist in Bloomingdale, IL. Duly provides therapists with the time, support, and resources to build their desired practice, emphasizing quality over volume. Therapists have dedicated patient time, access to a strong internal referral network, and opportunities for clinical specialty development. The role involves no double booking and no required marketing, allowing more focus on patient care. The Bloomingdale team is an established outpatient rehabilitation team supporting various patient populations and specialty areas like sports medicine, pelvic health, vestibular rehabilitation, and pediatrics. The role involves working alongside other healthcare professionals while maintaining autonomy for individualized, one-on-one care.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current or eligible Illinois Physical Therapist license.
- Must work minimum 2 hours a week.

Responsibilities
- Provide outpatient physical therapy services.
- Conduct 60-minute evaluations.
- Conduct 30-minute follow-up visits.
- Provide individualized, one-on-one care.
- Document patient care.
- Collaborate with PTs, OTs, PTAs, physicians, and advanced practice providers.
- Work a minimum of two Saturdays per month.
- Provide weekday availability, preferably.
- Work a minimum of 4 hours per shift.
- Work a minimum of 2 hours per week.
