As a Physical Therapist with Elara Caring, you’ll be a vital part of our mission by delivering high-quality, patient-centered physical therapy in the home. This role is hands-on and dynamic, requiring full-body movement, patient handling, and flexibility—including potential irregular or on-call hours.
Requirements
- Graduate of an APTA-approved Physical Therapy program
- Current, unrestricted PT license in the state of practice
- Minimum 1 year of experience in a healthcare setting
- Ability and willingness to travel up to 50% within the coverage area
- Valid driver’s license & auto insurance
- Ability to lift 50–100 lbs and perform physical job demands
Responsibilities
- Delivering high-quality, patient-centered physical therapy in the home
- Evaluating functional needs and developing customized plans of care
- Collaborating with physicians, caregivers, and interdisciplinary teams
- Adjusting care plans to optimize patient independence and outcomes
- Leading and mentoring support staff toward patient success
- Maintaining accurate, timely documentation and compliance standards
- Promoting physical therapy excellence within the community
