About The Position

As a Physical Therapist, you will perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. You will plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. You will evaluate the effects of treatment at various stages and adjust treatments to achieve maximum benefit. You will apply concepts of infection control/universal precautions in performing patient care, assure continuity of quality patient care delivered with appropriate documentation, and provide effective communication to patient/family, team members and other health care professionals. You will also promote agency philosophy and administrative policies and maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance

Nice To Haves

• 1 year of home health experience preferred
• HomeCare HomeBase experience preferred
