About The Position

Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care in Bergen County, NJ. This is an opportunity to join a growing organization within a progressive, supportive, and innovative team. All levels of experience are welcome, and training will be provided for the right new graduate. The role offers flexibility and autonomy, allowing you to see patients seven days a week, with a preference for treating patients in your primary geographic area. The company focuses on patient care with a quick EMR system (no Oasis required) and emphasizes a great work/life balance.

Requirements

Physical Therapist Program Graduate
Licensed Physical Therapist in the state of New Jersey or eligible.
Ability to travel to patients’ homes within the assigned geographic territory

Nice To Haves

Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
First Aid, AED, & CPR Certifications.

Responsibilities

Accept and treat patients within your designated geographic area
Provide 1:1 treatment
Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
Educate family members, home health aides, and other caregivers on proper therapy techniques
Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
Provide timely and accurate documentation for each visit
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
Must be willing to treat geriatric patients.
