About The Position
Duly Health and Care is seeking a PRN Physical Therapist to join our growing Rehabilitation Services team in our Glen Ellyn clinic. This role is ideal for clinicians seeking schedule flexibility, supplemental income, or an opportunity to remain active in outpatient practice while working alongside a collaborative team of rehabilitation professionals. Duly is expanding rehabilitation services across the Chicagoland area and is looking for therapists who can help support patient access and deliver exceptional care. Requirements
- Graduate of an accredited Physical Therapy program
- Current IL Physical Therapist license, or eligibility to obtain licensure
- CPR certification preferred

Responsibilities
- Evaluate & treat patients with orthopedic & musculoskeletal condition
- Develop individualized treatment plans & goals
- Collaborate with physicians & rehabilitation colleagues to ensure coordinated care
- Document patient care accurately within the EMR
- Deliver exceptional patient experiences focused on outcomes & quality care
