About The Position

Vertis Therapy is seeking a Full Time or Part Time Physical Therapist in the Ft Wayne, IN area. This role offers a great opportunity to join an experienced and collaborative therapy team. The position provides flexible scheduling and therapist autonomy, with a mentorship program available for new graduates or those transitioning to a new setting. Requirements

  • Master of Science degree in Physical Therapy or Bachelor of Science degree in Physical Therapy and five years of direct practice experience.
  • Current license/certification in the state of practice.
  • Prefer minimum one (1) year experience in rehabilitation.
  • Supervision, training and/or experience in a setting serving the same age/type of patient population served by this facility.
  • Prefer membership in the American Physical Therapy Association.
  • Demonstrated competency in identified areas of specialization.
  • Excellent oral and written communication and interpersonal skills.
  • Adequate computer skills for job duties and computerized documentation.
  • Demonstrated competency in applying principles, methods, materials, and equipment used in physical therapy.
  • Knowledge of accreditation standards and compliance requirements.

Responsibilities

  • Evaluates and treats patients in accordance with physician's orders using therapeutic procedures to address physical function deficits and improve independence, safety, and quality of life.
  • Manages the delivery of care throughout the treatment course, ensuring quality and efficiency.
  • Delegates appropriate components of care to the Physical Therapist’s Assistant under supervision.
  • Participates as a member of the interdisciplinary team, providing support and information within the scope of physical therapy.
  • Reviews patient information and assesses its impact on the assessment and treatment process.
  • Conducts and documents thorough functional assessments, including home/vocational and equipment needs.
  • Develops and implements individualized treatment plans with achievable functional goals.
  • Complies with evaluation, treatment, and documentation guidelines.
  • Records patient progress at least every seven (7) days.
  • Revises treatment plans as needed.
  • Documents the course of treatment and discharge summary clearly and concisely.
  • Instructs Assistants in treatment procedures and provides supervision.
  • Orients patients to program services and treatment procedures, informing them of potential risks.
  • Collaborates with nursing staff, patients, and families to maximize patient functional abilities.
  • Establishes and instructs patients and families/caregivers in exercises and therapeutic procedures for post-discharge continuation.
  • Assists in the referral process for patients requiring additional services post-discharge and maintains knowledge of community resources.
  • Attends and participates in rehabilitation team conferences.
  • Attends other department/facility meetings as required.
  • Accesses continuing education opportunities to maintain clinical competency.
  • Participates in the quality improvement process.
  • Acts in compliance with Vertis Therapy Management policies, State, Federal, and Professional Organization standards.
  • Provides input on the clinical performance of assistants to the Rehab Director as requested.
  • Promotes Vertis Therapy's programs and services.
  • Performs other duties as assigned.
