About The Position

Activa Home Health is seeking a clinically strong PRN Physical Therapist (PT) to support our Medicare Skilled Patients and our Hospital at Home Program. Provide Medicare/Oasis and hospital-level PT care in patients’ homes focusing on mobility, balance, fall prevention, and discharge readiness. Requirements

  • Active Physical Therapist (PT) license in the state of Florida
  • Degree from an accredited Physical Therapy program (DPT preferred)
  • Current CPR certification
  • Valid driver’s license and reliable transportation
  • Strong clinical reasoning, communication, and time-management skills

Nice To Haves

  • Experience in one or more of the following: Acute care or hospital-based physical therapy, Hospital at Home or transitional care programs, Medicare Skilled home health physical therapy
  • Skilled in: Gait training and balance rehabilitation, Fall risk assessment and prevention, Functional mobility and discharge planning, Post-acute and medically complex patient care
  • Comfortable practicing independently in a home-based acute care setting
  • Familiarity with EMR documentation systems

Responsibilities

  • Provide skilled physical therapy services to patients receiving hospital-level care at home and for Medicare Skilled Patients.
  • Perform comprehensive PT evaluations addressing: Strength, balance, endurance, and functional mobility, Gait deviations and fall risk, Transfers, bed mobility, and stair negotiation, Activity tolerance and cardiopulmonary response.
  • Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness.
  • Address functional decline related to: Recent hospitalization, Acute illness or injury, Deconditioning and mobility loss.
  • Educate patients and caregivers on: Safe mobility techniques and transfer strategies, Fall prevention and home safety, Use of assistive devices and mobility equipment.
  • Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team.
  • Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions.
  • Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards.
