Requirements

• Graduate of an accredited physical therapy educational program.
• Current Physical Therapist / PT state license within the state of practice.
• Current CPR certification.
• Excellent organizational, time-management, written and verbal communication skills.
• Reliable transportation, valid driver’s license and current automobile insurance.

Responsibilities

• Conducting one-on-one patient care in the home setting.
• Patient and family teaching in the home setting.
• Working directly with patients in their homes.
• Assessing patient needs.
• Developing personalized treatment plans.
• Implementing therapeutic interventions to optimize functional abilities.
