About The Position

Join our team as a Physical Therapist (PT) to develop and implement a progressive rehabilitation program for patients in their homes. This role involves serving as a case manager, supervising home health aides and physical therapist assistants, evaluating patients, planning and implementing treatment, performing reassessments, and discharging patients when goals are met or home health requirements are no longer met. You will also develop care plans for Home Health Aides and Physical Therapy Assistants, delegate duties, and supervise these team members according to agency policy. The position requires assuming responsibility for case management, consulting with community and internal resources, making interdisciplinary referrals, and performing patient care duties based on specific needs, including medical condition, age, and developmental status.

Requirements

  • Master’s Degree or Doctorate Degree in Physical Therapy
  • Have successfully completed and graduated from an accredited occupational therapy professional program recognized by NBCOT and have completed all fieldwork requirements
  • Current medical/health screening including 2 step TB clearance.
  • Valid State of Hawaii license to practice as an Occupational Therapist.
  • Current Basic Life Support CPR (cardiopulmonary resuscitation) certification through the American Heart Association
  • Two years of experience as an physical therapist, preferably in home health.

Responsibilities

  • Develop and implement a progressive rehabilitation program for patients in their homes.
  • Serve as case manager.
  • Supervise home health aides and physical therapist assistants when appropriate.
  • Evaluate assigned patients and admit those who meet agency and regulatory criteria.
  • Plan, coordinate, and implement appropriate treatment according to patient’s needs.
  • Perform ongoing reassessments.
  • Discharge patients when goals are met, or patient does not meet home health requirements.
  • Develop Home Health Aide (HHA) and Physical Therapy Assistant (PTA) care plans.
  • Delegate duties to HHA’s and PTAs based on agency and regulatory policies.
  • Supervise and provide input to HHA's and PTAs per agency policy.
  • Assume responsibility for case management of assigned patients.
  • Consult with appropriate community and internal resources and make appropriate interdisciplinary referrals.
  • Perform patient care duties and/or treatments based on specific needs, including medical condition, age, and/or developmental status.
