About The Position

This is a 13-week assignment for a Physical Therapist in a Skilled Nursing Facility (SNF). The role guarantees 35 hours per week, with day shifts from 8:00 AM to 4:30 PM. The Physical Therapist will be responsible for evaluating, treating, and monitoring residents to enhance their mobility, strength, balance, endurance, and overall functional independence. Collaboration with the interdisciplinary rehabilitation team is key to achieving optimal patient outcomes.

Requirements

• Active Physical Therapist (PT) license for the applicable state
• Minimum 2 years of Physical Therapy experience
• Current BLS certification
• Strong knowledge of therapeutic exercise, gait training, transfer techniques, and mobility rehabilitation
• Ability to develop individualized treatment plans and document patient progress
• Excellent communication, organization, and interdisciplinary collaboration skills

Nice To Haves

• Previous Skilled Nursing Facility (SNF) experience preferred
