About The Position

Avita Health System is seeking a dedicated Home Health Physical Therapist to join their OP Therapy Department at their Bucyrus location. The role involves evaluating, planning, directing, and administering physical therapy treatments as prescribed by a physician to restore function and prevent disability. The Physical Therapist will assist patients in reaching their maximum performance and functioning level, learn to live within their capabilities, and coordinate/delegate responsibilities to supportive staff. They will also participate in the interdisciplinary treatment team, assist with operational activities such as budget control, performance improvement, infection control, and department safety oversight.

Requirements

Graduate of an accredited Physical Therapy program and possess a Bachelor’s degree in Physical Therapy.
Current State of Ohio license as a Physical Therapist.
BLS certification of obtained within thirty (30) days of hire.

Nice To Haves

Minimum of one (1) year experience as a Licensed Physical Therapist.

Responsibilities

Evaluation, planning, directing and administering physical therapy modalities of treatment as prescribed by a licensed physician.
Administering treatments and physical agents as prescribed in an effort to restore function and prevent disability following injury, disease or physical disability.
Assisting patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.
Coordinating, delegating, and supervising responsibilities assigned to supportive staff (PTA and Therapy Aide).
Participating as a member of the interdisciplinary treatment team in planning and implementing the patient plan of care.
Assisting with operational activities including budget control, performance improvement, and infection control and department safety oversight.
