About The Position

Nightingale Home Healthcare, a provider with over 25 years of experience, is seeking a dedicated Physical Therapist to join their growing team. They focus on combining compassionate, patient-centered care with innovative technology to help individuals maintain independence, dignity, and quality of life in their homes. This role offers clinical independence with strong support, allowing therapists to practice with autonomy and flexibility, aiming for meaningful patient outcomes.

Requirements

• Current Physical Therapist license in Connecticut
• Minimum of 1 year PT experience
• Physical exam within the last 12 months
• Valid TB test (within 12 months) and current CPR certification
• Valid driver’s license and proof of auto liability insurance
• Ability to attend a 1–2 day orientation in Stamford, CT
• Availability Monday through Friday, 8:00 AM – 5:00 PM

Nice To Haves

• Home Health and OASIS experience preferred
• Experience with WellSky (Kinnser) software preferred

Responsibilities

• Evaluate patients and initiate therapy within 72 hours of referral
• Develop and implement individualized Plans of Care
• Supervise Physical Therapist Assistants (PTAs) and/or Home Health Aides (HHAs)
• Coordinate care with physicians, caregivers, and the interdisciplinary rehab team
• Maintain accurate documentation and timely visit notes
• Conduct supervisory visits and manage discharge planning
• Provide in-service education and participate in team meetings
• Ensure HIPAA compliance and adherence to agency policies
• Engage in ongoing professional development and collaboration
