Requirements
- Graduate of an accredited Physical Therapy program
- Current and active Physical Therapy license
- Current CPR certification
- Valid Driver’s License
- Current Auto Insurance
Nice To Haves
- Minimum 1 year of Home Health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Perform and document comprehensive initial evaluations and establish treatment diagnoses prior to intervention
- Develop and implement individualized treatment plans to maintain, improve, or restore patient function
- Evaluate patient progress and modify treatment plans to maximize outcomes
- Apply infection control and universal precautions during patient care
- Ensure continuity and quality of care through timely and accurate documentation
- Communicate effectively with patients, families, interdisciplinary team members, and healthcare professionals
- Support agency philosophy, goals, and administrative policies
- Complete all clinical documentation within company-established timelines
