Requirements
- Physical Therapist/DPT licensed in the State of Texas
- Skilled Nursing and Long Term Care Experience
- Cares for people and will go above and beyond to provide outstanding patient care
- Scheduling flexibility that allows you to do your best work while enjoying great quality of life
- Demonstrates a dedication for outstanding delivery of patient care and teaching
Nice To Haves
- EMR (PCC and NetHealth) is preferred
Responsibilities
- Implementation of physical therapy treatment plans of care.
- Coordination of care with entire care team
- Educating patients and family members
- Preparing timely, complete and accurate clinical notes
- Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members
