About The Position

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Nevada. Our ultimate mission is to dramatically improve patient access to home health care. The PT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs. We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time. If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction.

Requirements

Active Nevada PT license (in good standing)
Strong clinical judgment and comfort working independently in the field
Comfortable escalating changes in condition to RN/MD and documenting rationale clearly
Strong communication and documentation
Reliable follow-through on orders, frequencies, and patient goals
Current driver’s license and comfort with daily travel across assigned territory
Able to carry/transport basic therapy equipment as needed
Competency in core home health PT scope
Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations
Strengthening, functional mobility, and home safety assessment/modification education
Patient/caregiver coaching with practical home programs

Nice To Haves

Prior home health experience
Deeper clinical experience in high-signal PT populations
Experience supervising/mentoring PTAs and collaborating on care plans
Strong organization and time management

Responsibilities

Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.
Complete comprehensive in-home assessments and deliver skilled PT visits.
Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.
Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.
Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.
Manage treatment and outcomes directly, including PTA supervision where appropriate.
Coach patients and caregivers with practical home programs that actually get done.
Document promptly and thoroughly — defensible, efficient, and consistent.
