About The Position

Ultimate Therapy is seeking a Physical Therapist (PT) to join our rehabilitation team at Autumn Lake at Scotch Plains. This position provides the opportunity to work with residents in a skilled nursing and post-acute care setting, helping them improve mobility, function, and overall quality of life. Autumn Lake at Scotch Plains has an established rehabilitation program serving both short-term rehabilitation and long-term care residents. We foster a collaborative environment with PT, OT, SLP, and nursing professionals, offering supportive clinical leadership and strong interdisciplinary communication with a focus on resident-centered care and meaningful functional outcomes.

Requirements

Active Physical Therapist license required.
Strong clinical reasoning and communication skills.

Nice To Haves

Skilled nursing experience preferred but not required.
New graduates are encouraged to apply.

Responsibilities

Conduct physical therapy evaluations and develop individualized treatment plans based on resident needs and goals.
Provide therapy services focused on mobility, gait training, balance, strengthening, endurance, transfers, and pain management.
Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to coordinate resident care.
Monitor resident progress and adjust treatment approaches as appropriate.
Educate residents and families on therapy goals, exercises, and safety strategies.
Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
