About The Position

Therapy Partner Solutions (TPS), a therapist-owned and operated company since 2016, is seeking a Physical Therapist for home health. As the provider of home health therapy services for Accent Care Home Health, we are part of a strong, multidisciplinary team dedicated to providing exceptional therapy services. We understand the unique challenges clinicians face and are committed to supporting our team members. Requirements

Graduate (current or pending) from an accredited physical therapy program.
Active or pending state physical therapist license.
Comfort working independently in the home setting.

Responsibilities

Provide physical therapy services to patients in their homes.
Work with a diverse and engaging caseload.
Collaborate with a multidisciplinary team, including physicians and other healthcare professionals, for coordinated, patient-centered care.
Utilize thoughtful scheduling and defined territories to manage caseloads.
Minimize drive time between visits whenever possible.
Engage in individualized clinical and professional development.
Complete documentation training to ensure confidence and efficiency.
