About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve San Anselmo and the surrounding areas. As an independent contractor, you are your own boss and control your own schedule. FeldCare Connects provides administrative support, including assigning, communication, scheduling, care coordination, and quality assurance. Clinicians have the flexibility to see patients when and where they want, with no minimum requirement. The role involves one-on-one treatment, where the therapist observes, records, and reports on patient response to treatment and changes in condition, and determines the plan of care. Documentation is made easy through an intuitive Electronic Medical Record system.

Requirements

• Current Physical Therapist license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using the Electronic Medical Record system.
