About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in La Crescenta and surrounding areas. The position is for an independent contractor working through FeldCare Connects, an app-based referral network. Contractors choose their own schedule and preferred coverage areas, with no minimum patient requirement, and can accept or decline patient referrals through the Cliniconnects app. Compensation is based on per-visit rates and the number of patients treated.

The network provides administrative support with assignments, communication, scheduling, care coordination, and quality assurance. The therapist provides one-on-one treatment, determines the plan of care, and observes, records, and reports patients’ responses to treatment and changes in their condition. Documentation is completed through the network’s electronic medical record system.

FeldCare Connects links clinicians with home health agencies and hospices to help homebound patients access care. The network contracts with more than 900 home health agencies across California, Arizona, Texas, Nevada, Idaho, Hawaii, and Florida, and connects more than 2,000 clinicians with patients in their communities.

Requirements

• Current, active, and unrestricted Physical Therapist license issued by the state board of Physical Therapy; conditional, provisional, restricted licenses, and limited permits are not accepted.
• Completion of an accredited Physical Therapy program.
• Ability and enthusiasm to work with patients of all ages.
• Reliable personal transportation.
• Smartphone.

Responsibilities

• Provide quality home healthcare and one-on-one treatment to patients of all ages.
