About The Position

Signature Healthcare at Home, part of The Pennant Group, is seeking a Physical Therapist (PT) for their Lincoln City, Oregon location. This field-based role involves providing one-on-one care to patients in their homes to help them regain mobility, independence, and quality of life. Requirements

Licensed Physical Therapist (PT) in Oregon (or eligible)
Graduate of an accredited Physical Therapy program
Valid driver’s license and reliable transportation
Ability to work independently in a field-based setting

Nice To Haves

Prior home health experience
Familiarity with OASIS and Medicare documentation
Experience serving rural or geographically spread communities

Responsibilities

Evaluate and treat patients recovering from injury, illness, or surgery.
Develop and implement individualized plan of care.
Improve patient mobility, strength, balance, and functional independence.
Provide interventions focused on fall prevention and safety.
Collaborate with physicians, nurses, and interdisciplinary team members.
Adjust treatment plans based on patient progress.
Participate in case conferences and care planning.
Teach patients and families exercises, safety strategies, and mobility techniques.
Empower patients to manage their conditions at home.
Complete accurate and timely clinical documentation (OASIS experience preferred).
Follow all Medicare home health regulations and Oregon state requirements.
Support improved patient outcomes and satisfaction.
Reduce rehospitalizations and promote safe recovery at home.
