About The Position

Elite Home Health is a Medicare-certified home health agency serving communities across Florida. Requirements

Graduate of an accredited Physical Therapy program required.
Current state Physical Therapist (PT) license in good standing required.
Minimum of one (1) year of experience in physical therapy required.
Current CPR certification required.
Valid driver’s license, reliable transportation, and auto insurance.
Ability to pass background screening and all compliance requirements.
Ability to stand, walk, bend, lift, and assist patients as needed.
Ability to perform therapy assessments and interventions in home environments.
Ability to use electronic documentation systems and therapy equipment.
Ability to communicate effectively with patients, families, and care team members.

Nice To Haves

Home health, acute care, or rehabilitation experience preferred.

Responsibilities

Provide skilled physical therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations.
Evaluate patient mobility, strength, balance, and functional status.
Develop and implement therapy interventions.
Work collaboratively with the interdisciplinary care team to improve safety, independence, and quality of life.
Perform initial and ongoing patient evaluations to assess mobility, strength, balance, endurance, and functional limitations.
Complete comprehensive assessments and participate in OASIS-related coordination as applicable.
Develop, implement, and update individualized physical therapy treatment plans.
Ensure physician orders are appropriate and aligned with the patient’s therapy needs.
Guide patients through therapeutic exercises, gait training, transfer training, balance activities, and mobility restoration.
Instruct patients in the safe use of assistive devices including walkers, canes, wheelchairs, and other mobility equipment.
Support fall prevention, pain management, and restoration of safe functional movement in the home setting.
Teach and counsel patients and caregivers regarding therapy goals, home safety, fall prevention, mobility strategies, and self-care techniques.
Evaluate the home environment and identify safety modifications or community resources needed to support care.
Coordinate outpatient services when procedures cannot be safely performed in the home.
Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries.
Evaluate effectiveness of therapy interventions and update the plan of care as needed.
Maintain compliance with Medicare requirements, state regulations, and agency policies.
Communicate regularly with physicians, nurses, and interdisciplinary team members regarding patient progress and needs.
Supervise the Physical Therapist Assistant (PTA) in accordance with agency policy and regulatory requirements.
Supervise the Home Health Aide when Physical Therapy is the only skilled discipline involved in patient care.
Participate in care conferences, discharge planning, staff meetings, and required in-services.
