About The Position

Reviews all relevant outpatient medical records, imaging, and physician notes prior to initiating treatment. Evaluates patients thoroughly to establish individualized care plans, functional goals, and expected timelines for improvement. Implements evidence-based outpatient therapeutic interventions including exercise prescription, manual therapy, functional training, and modalities. Ensures safe and appropriate patient handling techniques during all treatment sessions. Provides timely, accurate, and compliant outpatient documentation in accordance with departmental and regulatory requirements. Educates patients and families on home exercise programs, symptom management, and expectations for progression. Maintains close, consistent communication with orthopedic and family health center providers regarding patient progress, response to treatment, and recommendations. Participates in interdisciplinary discussions regarding outpatient orthopedic cases and continuity of care. Collects outcome measures, patient data, and quality metrics that support departmental Performance Improvement (PI) reporting. Maintains open communication with outpatient therapy team members to ensure continuity of care across schedules. Attends relevant continuing education programs to maintain clinical competency and required CEUs. Maintains flexibility to work some early mornings and later evenings to support patient scheduling needs.

Requirements

  • B.S., M.S., or DPT in physical therapy
  • NYS licensure
  • Ability to initiate a physical therapy care plan and program and establish short term and long-term goals for the individual.
  • Excellent communication and interpersonal skills.
  • Must be competent and caring for assessment and treatment of patients of all ages.
  • Adaptable in learning new EMR system and maintaining compliance through documentation.
