About The Position

The Physical Therapist works with a PACE (All-Inclusive Care for the Elderly) program serving seniors who wish to continue living in their homes and communities. The organization uses a collaborative interdisciplinary team (IDT) approach to evaluate participants and guide their care. The therapist assesses participants’ physical therapy needs, preferences, and goals in the facility and at home; participates in IDT meetings to develop care plans; and delivers and documents interventions in accordance with physician orders and participant care plans. The role also includes supervising Therapy Assistants and training staff, participants, and caregivers in therapeutic programs and equipment use.

Requirements

• Bachelor’s degree in Physical Therapy from an accredited program.
• Valid California PT License or proof of license eligibility.
• At least two years of experience with frail and/or elderly populations.
• At least two years of OTA experience in a clinical setting, as stated in the posting.
• Requisite knowledge to treat frail, elderly participants with complex conditions.

Nice To Haves

• Master’s degree in Physical Therapy or Doctorate in Physical Therapy.

Responsibilities

• Conduct Physical Therapy assessments in the facility and in participants’ homes to define care needs, preferences, and goals.
• Participate in IDT meetings to develop participant care plans.
• Deliver and document Physical Therapy interventions in accordance with physician orders and/or participant care plans.
• Provide active and passive exercise, muscle re-education, gait and functional ADL training, and treatments such as massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants and provide training, instruction, and guidance to implement Functional Maintenance Programs (FMP).
• Train and instruct staff, participants, and caregivers or family members in the use and care of therapeutic equipment.
