About The Position

North Florida Rehabilitation Hospital is seeking a Physical Therapist (PT) for a PRN position. As a Physical Therapist joining our team, you’re embracing a vital mission dedicated to making communities healthier. This role involves evaluating a patient's condition, developing a treatment plan, and helping them recover. You will also instruct nursing staff and patient families on follow-through programs, communicate patient progress, assist with scheduling and posting charges, and document patient care according to requirements. The role also includes maintaining equipment and work areas, and adhering to the Company’s Code of Business Conduct and other applicable laws.

Requirements

Degree from an accredited Physical Therapy program
Current and unrestricted Physical Therapy license in the state where services are rendered
Current CPR certification
Strong organizational and communication skills

Responsibilities

Evaluate a patient's condition, develop a treatment plan, and help them get better.
Instruct the nursing staff and the patient's families on follow-through programs.
Communicate patient progress or problems to supervisor and other team members.
Assist with patient scheduling and post charges daily to patient records.
Document patient care in accordance with regulatory, licensing, payer and accrediting requirements.
Maintain equipment and work area in a safe and clean condition.
Handle job responsibilities in accordance with the Company's Code of Business Conduct, the Corporate Compliance Agreement, appropriate professional standard and applicable state/federal laws.
Other duties as assigned.
