About The Position

Aspire Home Healthcare is seeking a skilled Physical Therapist (PT) to join their team and help patients regain independence through one-on-one home health care. The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team. This role involves providing effective patient care using approved therapy techniques and procedures, evaluating patient condition, establishing measurable treatment goals, and developing and implementing individualized treatment plans. The PT will also supervise and coordinate care with PTAs and HHAs, document patient progress, educate patients and families, evaluate home environments, and ensure services meet HIPAA standards. The position offers flexible scheduling, supportive leadership, and a patient-first environment.

Requirements

  • Active Physical Therapist (PT) license in OH
  • Minimum 1 year of PT experience preferred
  • Current TB test (within 12 months) and CPR certification
  • Valid driver’s license and reliable transportation

Responsibilities

  • Provide effective patient care using approved therapy techniques and procedures
  • Evaluate patient condition and establish measurable treatment goals
  • Develop and implement individualized treatment plans
  • Supervise and coordinate care with PTAs and HHAs
  • Document patient progress, updates, and treatment notes accurately and on time
  • Educate patients and families on exercises, mobility, and use of adaptive equipment
  • Evaluate home environments and recommend modifications to improve safety and independence
  • Ensure all services meet HIPAA standards for confidentiality
  • Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
