As a home care Physical Therapist, you will be an integral member of a multi-disciplinary team that provides rehabilitative care to clients, allowing them to live independently and age in place safely. Requirements

Current Minnesota Physical Therapist license
Graduation from an accredited physical therapy program as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills: prior experience with electronic medical records (EMR) preferred.

Responsibilities

Evaluate and treat clients suffering from physical disability due to injury, disease, or surgery in a home care environment
Provide treatment for clients using a variety of methods including exercise, hands-on therapy, stretching maneuvers, and equipment to ease clients' pain and help them increase their mobility
Test, measure, and assess clients' current condition and abilities
Assist patients with all phases of Physical Therapy treatment and techniques to include exercise instruction, manual therapy techniques, activities of daily living, functional/total body progressions, modalities, and a fall prevention program
Establish treatment plans specific to the client's specific needs that allows for excellent client outcomes
Make recommendations for devices or tools that might improve the client's quality of life
Work cooperatively and consistently with physicians, case managers, nurses, occupational therapists, and caregivers
Provide training to team members as needed
Request interventions to help avoid unnecessary trips to the emergency room or readmissions
Monitor, document, and report clients' progress
