About The Position

The Physical Therapist evaluates patients’ clinical needs and initiates, develops, implements, and revises plans of care, addressing and documenting all identified problems. The role uses HHRG values to establish visit frequency and appropriate interdisciplinary utilization, and works toward annual outcomes that meet National Benchmarks. The Physical Therapist participates in interdisciplinary patient case conferences, evaluates the effectiveness of care plans, coordinates appropriate discharges, and communicates relevant changes in patient condition to physicians and other care providers. The position also includes timely and complete clinical documentation, obtaining and clarifying valid physician orders, and providing service consistent with VNA’s CARES values: compassion, accountability, respect, excellence, and service.

Requirements

Bachelor of Science or Master’s degree in Physical Therapy from an institution accredited by the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
Current Massachusetts Physical Therapist license.
One year of clinical experience.
CPR certification.

Responsibilities

Maintain annual HHRG values at the National Standard as appropriate.
Use HHRG values to establish visit frequency and appropriate interdisciplinary utilization.
Meet annual outcomes benchmarks.
Refer patients to community resources as appropriate.
Evaluate patients’ clinical needs and initiate, develop, implement, and revise plans of care, addressing and documenting all problems.
Participate in interdisciplinary patient case conferences and evaluate the effectiveness of established plans of care.
Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.
Notify physicians and other involved care providers of significant changes in a patient’s condition.
Complete admission and other paperwork in a timely manner.
Document skilled care clearly and concisely, including details of teaching, direct care, and the patient’s or caregiver’s response.
Obtain and clarify valid physician orders for all care delivered.
Plan and coordinate discharges, inform the patient and physician, record the patient’s disposition in the discharge summary, and complete required discharge paperwork.
Provide service excellence to patients, family members, visitors, volunteers, and coworkers in accordance with VNA’s CARES values.
