About The Position

The Physical Therapist at VNA of Cape Cod in Nantucket is responsible for evaluating the clinical needs of patients, initiating, developing, implementing, and revising their plans of care. This role involves ensuring annual HHRG values meet National Standards and Benchmarks, utilizing these values for visit frequency and interdisciplinary utilization, and referring patients to appropriate community resources. The therapist participates in inter-disciplinary patient case conferences, evaluates the effectiveness of care plans, and provides documented supervision for Home Health Aide plans of care every 14 days. Key duties also include notifying medical doctors and other care providers of significant changes in a patient's condition, completing all admission and discharge paperwork in a timely manner, and recording narrative documentation that reflects skilled care, teaching, direct care, and patient/caregiver response. The Physical Therapist must obtain and clarify valid MD orders for all care delivered and consistently provide service excellence in line with VNA’s CARES commitment.

Requirements

  • Bachelor of Science Degree or Master's Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association.
  • Current PT license in Mass.
  • One (1) year clinical experience.
  • CPR certification required.

Responsibilities

  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and to establish appropriate interdisciplinary utilization.
  • Annual outcomes meet National Benchmarks.
  • Refers to community resources as appropriate.
  • Evaluates the clinical needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care; addresses all problems in the plan of care and documents accordingly.
  • Participates in inter-disciplinary patient case conferences; evaluates effectiveness of established plan of care.
  • Instructs and provides documented supervision of the Home Health Aide plan of care every 14 days.
  • Notifies MD/other involved care providers of significant changes in the patient’s condition.
  • Completes admission and other paperwork on a timely basis.
  • Records narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  • Obtains and clarifies valid MD orders for all care delivered.
  • Plans and coordinates discharges appropriately and informs patient/MD and records disposition of patient in the discharge summary. Completes all appropriate discharge paperwork.
  • Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service.
