About The Position

FOX Rehabilitation is expanding in South Plainfield, NJ, and is seeking dedicated, licensed Physical Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. The role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. This position offers autonomy, flexibility, and the opportunity to build strong patient relationships while managing your own treatment schedule. Clinicians can also integrate telehealth and virtual care for increased flexibility and patient support. This is a high-impact PT role designed for those who wish to positively influence the lives of the communities served.

Requirements

  • Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
  • Degree from an accredited Physical Therapy program.
  • Basic computer and EMR proficiency.
  • Current CPR certification.

Responsibilities

  • Provide one-on-one physical therapy to older adults in private residences, Senior Living Communities, Independent Living, Assisted Living, and Memory Care settings.
  • Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
  • Deliver therapy sessions focused on patient wellness, longevity, mobility, and functional independence.
  • Document effectively within FOX’s EMR system in accordance with regulatory standards.
  • Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
  • Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
  • Provide telehealth / virtual care when appropriate to extend access and flexibility.
