Physical Therapist (PT) – Flexible Schedule
About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) who desires more control over their schedule, meaningful patient relationships, and the ability to increase their income based on productivity. This role offers flexibility and autonomy, allowing you to create and manage your own schedule. You will provide 1:1 patient care in their homes, with AI-assisted charting to reduce documentation time and a user-friendly EMR system. The position includes structured onboarding, mentorship, ongoing training, free CEUs, and career advancement opportunities. Coverage is within The Villages, FL 32159 / Sumter County, FL, with territories designed to minimize drive time.
Requirements
- Active, unencunmbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
