About The Position

Argus Home Health Care is seeking a Physical Therapist (PT) for their Home Health division. The company has been providing a wide array of medical services in the home environment since 1990, focusing on preventative and restorative medical care with compassion and respect. The PT will provide physical therapy services in clients' homes as prescribed by a physician and offer consultation and education to agency staff. This role works under agency policies and client care plans, reporting to the Argus of Colorado administrator or designee, and supervises assigned personnel.

Requirements

Active Colorado PT license
Graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) or any accrediting body approved by the U.S. Office of Education
Valid Driver's License & clean Motor Vehicle Record (MVR)

Experience in Home Health preferred, but not required

Responsibilities

Conduct initial and ongoing evaluations, including OASIS assessments.
Develop and update care plans with the physician and care team.
Identify barriers, set goals, provide treatment, education, and equipment.
Coordinate services and plan discharge.
Evaluate care effectiveness.
Record evaluations, treatments, and client responses promptly.
Document conference outcomes and quality assurance data.
Attend care conferences, staff meetings, and in-service programs.
Provide education and consultation to clients, families, and staff.
Supervise aides, therapy assistants, and paraprofessionals.
Maintain professional knowledge through education, professional organizations and workshops.
Perform other related duties as needed.
