About The Position

A Physical Therapist (PT) is needed for a PRN (as-needed) Home Care position in Cleveland, OH. Enjoy a great work-life balance with a growing Home Care Company in Cleveland, OH. This is a PRN position with flexible scheduling. We are looking for coverage across different areas, including the west side of Cleveland and Mentor.

Requirements

• Current state license to practice as a Physical Therapist in Ohio.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong communication, organizational, and time-management skills.

Nice To Haves

• Prior home care experience preferred.

Responsibilities

• Develop treatment plans by identifying a problem list and methods to achieve identified goals.
• Coordinate treatment plan with the patient, family, physician, and other team members.
• Adapt treatment to meet the individual needs of the patient.
• Review the quality and appropriateness of the total services delivered.
• Supervise PTAs and HHAs.
• Complete clinical documentation in a timely manner.
