About The Position

The Physical Therapist will evaluate patients, select and administer assessment techniques, and synthesize information to establish an appropriate patient care plan. This includes necessary modalities, procedures, exercise programs, therapeutic groups, and patient education based on the patient's diagnosis, condition, age, and discharge needs. The role involves gathering and interpreting data from medical records, physician reports, and patient/family input. Functional goals will be established in conjunction with the patient, and evidence-based interventions will be provided with ongoing assessment to adjust the plan of care as needed. Documentation of evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges is required. Communication of patient evaluations and progress to physicians, referral sources, and other clinical staff is essential, along with coordinating patient schedules, treatment programs, and discharge planning. The Physical Therapist will also provide supervision of PTA's and Rehab Tech/Aides as required for licensure.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • Applicable state license for Physical Therapy required.
  • BLS required per policy guidelines.
