Requirements

Graduate of an accredited Physical Therapy program.
Current Connecticut Physical Therapist license.
At least 1 year of experience as a practicing Physical Therapist.
Strong communication and interpersonal skills with a compassionate approach to patient care.
Ability to work independently, manage priorities, and organize a flexible patient schedule.
A genuine passion for helping patients achieve greater mobility and independence.

Nice To Haves

Home Health Physical Therapy experience preferred.

Responsibilities

Provide high-quality, patient-centered physical therapy services in patients’ homes.
Evaluate patients’ mobility, strength, functional abilities, and rehabilitation needs.
Develop and implement individualized treatment plans focused on improving independence and quality of life.
Monitor patient progress and adjust treatment plans to achieve the best possible outcomes.
Educate patients and families on exercises, therapy techniques, safety, and lifestyle modifications.
Collaborate with nurses, physicians, therapists, and other healthcare professionals to provide coordinated care.
Maintain accurate and timely documentation of patient visits and progress.
