About The Position

Dove Healthcare is currently looking to add a Physical Therapist to our dynamic therapy team! Travel required between the Bloomer campus and the Regional Vent Center in Chippewa Falls. This is approximately a 20-hour per week position with a flexible schedule and adaptable days and hours. Mileage reimbursement is provided. Requirements

  • Doctorate degree (or bachelor's / master's degree for those grandfathered in) in physical therapy from an accredited college or university
  • Current license as a physical therapist in the State of Wisconsin

Responsibilities

  • Plan, develop, organize, implement, evaluate, and direct physical therapy, as well as its programs and activities, in accordance with current rules regulations, and guidelines that govern a skilled nursing facility
  • Participate in discharge planning, development and implementation of patient care plans, patient assessments, etc. as necessary
  • Maintain treatment records, patient files, and progress notes as required
  • Participate in the development and implementation of patient assessments and care plans, including quarterly reviews
  • Collaborate and supervise PTAs
