About The Position

FeldCare Connects is seeking an enthusiastic, self-motivated Physical Therapist to deliver quality home healthcare to patients of all ages. This position is for an Independent Contractor to serve Torrance and the surrounding areas. As an independent contractor, you are your own boss, controlling your own schedule and work in preferred coverage areas. You determine the plan of care and can observe, record, and report the patient's response to treatment and changes to the patient's condition. FeldCare Connects provides administrative support including assigning, communication, scheduling, care coordination, and quality assurance. They utilize an Electronic Medical Record system for documentation and an app called Cliniconnects for managing patient referrals.

Requirements

• Current Physical Therapist license issued by the state board of Physical Therapy.
• The required license MUST be current, active, and unrestricted; a conditional, provisional, or restricted license will NOT be accepted. Limited permits will not be accepted.
• Completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone.
• Ability and enthusiasm in working with all ages.

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Determine the plan of care.
• Complete documentation using the Electronic Medical Record system.
