About The Position
Human Touch Home Health is hiring a licensed Physical Therapist to provide home health services in Woodbridge, Dale City, Lake Ridge, Dumfries, Montclair, Occoquan, Lorton, Springfield, Alexandria, and surrounding communities north of Triangle along the I-95 corridor. This role offers flexibility, greater autonomy, and the opportunity to build meaningful relationships with patients.
Requirements
- Active Virginia Physical Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete evaluations, treatment plans, reassessments, and discharge planning
- Provide skilled therapy services in patients’ homes
- Coordinate care with physicians and the interdisciplinary team
- Manage your own schedule and daily route
